OrthoClast®2 System – Efficient and Bone-Preserving Revision Surgery

The OrthoClast2 system by endocon GmbH is an advanced solution designed for the efficient and minimally invasive removal of cementless prostheses. Utilizing pneumatic shock wave technology, it facilitates the dissection of the bone-prosthesis interface while preserving bone integrity and reducing the need for extensive surgical interventions.
Applications:
The OrthoClast2 system is suitable for various clinical scenarios, including:
- Revision surgeries involving cementless prostheses
- Cases with septic or aseptic loosening of prosthetic components
- Removal of broken or misaligned prosthesis stems
Improved functions of the OrthoClast2 system:
- Working pressure up to 6 bar

- Error detection and output on screen (e.g. missing compressed air)
- User-specific settings can be saved by the user (working pressure and frequency)
- Choice of language
- Touch display
- Operation with surgical gloves possible
- New quick coupling for hose connection

Cementless prosthesis revision
Revision of cementless prostheses with pneumatic shock waves

The removal of cementless prosthesis using the conventional technique with transfemoral access usually ends with major damage to the bone. The OrthoClast system in combination with the flexible micro chisel preserves the bone stock and normally avoids transfemoral access. The OrthoClast System works with shock wave technology comparable to a pneumatic hammer.
Field of use:
• Structured / unstructured prosthesis stems
• Septic / aseptic release of the prosthesis stem
• Broken / damaged prosthesis stems
• Misaligned prosthesis stems
The OrthoClast handset combined with a small, flexible micro chisel works up to a maximum depth of 190 mm.
• Dissection of the bone-prosthesis interface
• Chisel with pre-defined bone and prosthesis sides
• Controlled sliding of the flat chisel along the prosthesis stem
• No bone perforation or fissure

Advantages:
• Controlled sliding of the chisel
• Bone-saving method to dissect the bone-prosthesis interface
• No femoral fenestration
• Significantly reduced operation time
• Minimal bone trauma

How to use the OrthoClast - for beginners
Consumables for cementless revision
Flexible chisel - FR-078

Function: separate the interface between the prosthesis and the bone
Field of use: proximal to distal shaft area
Working length: 160 mm
Note: when driving in the flexible chisel, it is important to ensure that it is correctly aligned. For this reason, there are markings on the chisel blade to indicate which side must face the prosthesis or the bone when chiseling.
Flexible chisel - FR-079

Function: separate the interface between the prosthesis and the bone
Field of use: proximal to distal shaft area
Working length: 190 mm
Note: when driving in the flexible chisel, it is important to ensure that it is correctly aligned. For this reason, there are markings on the chisel blade to indicate which side must face the prosthesis or the bone when chiseling.
Flexible chisel - FR-077

Function: separate the interface between the prosthesis and the bone, suitable for Wagner and ribbed stems
Field of use: proximal to distal shaft area
Working length: 160 mm
Width (tip): 5 mm
Note: when driving in the flexible chisel, it is important to ensure that it is correctly aligned. For this reason, there are markings on the chisel blade to indicate which side must face the prosthesis or the bone when chiseling.
Split mallet - FR-062

Function: to hammer out the extraction sleeve with the flexible chisel.
Field of use: Extraction of flexible chisel jammed FR-080 between the prosthesis and the bone.
Extraction sleeve - FR-080

Function: for chisels with a shaft width of up to 5 mm. Positioned on the flexible chisel, the extraction sleeve acts as a support for the split mallet.
Field of use: extraction of flexible chisel jammed between the prosthesis and the bone.

Cement removal
Revision of cemented prostheses using endoscopic view

The manual removal of bone cement during cemented arthroplasty revision with conventional chisels is exhausting, imprecise and a mine field of potential complications. The OrthoClast system simplifies all processes, preserves the bone stock and usually avoids fenestration.
Field of use:
• Proximal bone cement removal for hip and knee revision arthroplasty
• Medial and distal bone cement removal with endoscopic view
• Removal of the distal cement tip and the medullary plug with endoscopic view
This method is based on decades of experience in the development and application of pneumatically generated shock waves in the medical field.

Advantages:
• Endoscopic view to the end of the medullary cavity
• Precise and controlled guiding of the chisel, significantly reduced danger of cortical bone perforation
• Removal of cement by saving the bone structure all the way down to the bottom
• Unimpaired, intact cortical bone; no cerclages necessary
• Minimal bone trauma
• Significantly reduced operation time
• Reduced convalescence period for the patient

OrthoClast Consumables cemented revisions
Chisel set for cemented stem revisions

The chisel set has been specially developed for the revision of cemented stem prosthesis with the OrthoClast system. The set consists of 12 different chisels, which have been specially designed for the requirements of the bone cement removal in the proximal, medial and distal area. The chisels differ in length, bend and through the polished section of the chisel tips. All set's chisels are reusable.
